About Highway Hospice
Highway Hospice is a registered non-profit organisation that has served Durban for over forty years, providing palliative and end-of-life care to patients with life-limiting illness and their families. Our 24-hour In-Patient Unit (IPU) will offer short-stay symptom control, respite and end-of-life care alongside our established Home-based service.
The Role/Key Responsibilities
We are looking for an experienced and mature Professional Nurse to join our Home Care Department, to provide palliative care to our home-based patients.
- To deliver high quality comprehensive palliative care to our home-based patients, diagnosed with a terminal illness, and their caregivers in their own homes by way of managing pain and symptoms.
- Provide psychosocial support to patients, their families and caregivers
- Develop and promote evidence-based palliative nursing care protocols across our HBC
- Formulate, implement, and evaluate individualized care plans for each patients and their caregivers.
- Maintain accurate patient records, documentation and medico-legal reports
- Handle, maintain and manage new patient referrals whilst working alongside Admissions office
- Demonstrate leadership skills to promote effective teamwork and caring for caregivers
- Demonstrate good communication skills, written and verbal, with staff patients, family members and caregivers
- Effective management of resources, human, technical and financial within the
- Home Care Department and Highway Hospice
- Collaborate with all members of the interdisciplinary team to deliver person centred care
Requirements & Qualifications
To be considered for this role, you must meet the following criteria:
Essential
- South African Nursing Council Registration
- Minimum of 5 years working experience as a Registered Nurse
- Valid driver’s licence for a manual car
- Good communication skills
- An ability to work well under pressure
- Palliative care experience and skills would be an advantage
